Embed a Paubox form on your website

Paubox Forms allows you to collect HIPAA-compliant submissions directly from your website using a simple embed code. Embedding forms ensures secure data collection while maintaining a seamless experience for your site visitors.

How to embed a form

1. Log in to your Paubox dashboard
2. Navigate to Forms 
3. Select a published form from your list
4. Click the Embed tab at the top of the screen
5. Click the clipboard icon in the code box to copy the single-line embed code
6. Paste the code into an HTML content block on your website

NOTE : Draft and archived forms cannot be embedded until they are published. If you attempt to access the Embed tab on an archived or draft form, the system will notify you that the form must be published or unarchived before generating code.

Form visibility and error handling

• Draft or archived forms: If a form is unpublished or archived, embedded instances will not load on your website. Unarchive or publish the form in your Paubox dashboard to restore visibility.
• Unavailable forms: If a form link fails to load or becomes inaccessible, visitors will see a clear message stating "This form isn't available" rather than a blank page.
