Exporting Paubox Forms submissions

Paubox Forms allows you to securely collect information and files from your patients or clients. Exporting your form submissions to CSV or PDF files makes it easy to review data, manage responses, and import information into other clinical systems.

 

How to export all submissions

  1. Log in to your Paubox dashboard

  2. Navigate to Paubox Forms in the left-hand navigation menu

  3. Locate the form you want to export and click the menu button on the right side of the table

  4. Select View submissions from the menu

  5. On the submissions page, click the Actions dropdown menu

  6. Select Download all submissions to generate and download the CSV file

 

How to export a single submission

  1. Log in to your Paubox dashboard

  2. Navigate to Paubox Forms in the left-hand navigation menu

  3. Locate the form and click the menu button, then select View submissions

  4. Locate the specific submission you wish to export

  5. Click the action button next to the submission

  6. Select Download PDF or Download CSV

 

Note: Attachments sent via your form are not included in the CSV or PDF downloads. To access these files, please refer to the form notification email sent to the designated form recipients.
